| This project was the company service portal of "Client" for patients suffering from sleep disorders. It was information about the company, key personnel, HIPPA privacy, education resources, directions and contact information. Forms, brochures and questionnaire were available for download in PDF format. There was a new user registration leading to patient information form and clinical questionnaire. Existing Users can login and update the sleep log. The home page features a GASP Questionnaire to provide a quick diagnosis to a site visitor based on common symptoms of sleep disorder. Finally there was an admin panel with a content management system so that the site administrator can easily change the site content and manage users / patient information. |

Forms, brochures and questionnaire were available for download in PDF format. There was a new user registration leading to patient information form and clinical questionnaire. Existing Users can login and update the sleep log. The home page features a GASP Questionnaire to provide a quick diagnosis to a site visitor based on common symptoms of sleep disorder. Finally there was an admin panel with a content management system so that the site administrator can easily change the site content and manage users / patient information. This was the core web form application and was written using ASP.Net with MySQL or MS SQL Server as database on a Windows 2003 server. The purpose of this application was patient data entry and consolidation. Cross browser compatibility was maintained spanning Internet Explorer, Netscape Navigator & FireFox. HTML 4compliance was the key to cross browser compatibility. We were followed the HIPPA guidelines regarding safekeeping of patient information. |

Visitor Capabilities Site The capabilities provided by the application to a site visitor can Search for appointment using application wide criteria including name of doctor, name of patient, date booked, date of appointment, associated properties, specialty and time slot among others. |

Administrator Capabilities The administration system was a secure password protected was a, which was provided access to other users. The super admin who has having access to the whole system and the other users have access to the specific was as on the site. The administrator was able to manage the entire site content, channels and site rules from the administration was a. It was having menus displayed at the top such as Setup, Manage, Home & Logout. |

Desktop Admin Capabilities This was a separate "Win Forms" application written in VB.Net with MS Access as database. It has a login authentication for the administrator and provides a synchronization facility to download any new or updated patient records from the web database. Once downloaded the patient records can be browsed locally in this application. Analysis and Reporting services was not envisaged in the current phase. In the remaining sections of the proposal, we have used these artifacts to create a work breakdown structure and assign roles and estimated person hours for each activity. Consolidated effort estimation, project schedule and price / payment terms was provided. |

The Web Form application and the Win Form Application together constitute a self contained system with little external integration envisaged at this point. The only external integration was to Google Maps. SSL Certificate was loaded on web server so that new and existing patient data was submitted securely to web server. |
